Family Medicine

Personalized, comprehensive care for every member of your family, from the energetic toddler to the wise elder.

Chronic conditions are managed alongside psychiatric and substance use treatment rather than referred out. For a household where more than one of those is in play, it is one practice and one record.

  • All ages

Who for

Every age

the whole household, one practice

What the practice covers

The routine and the urgent in the same place, for every age in the family.

  • Chronic disease management

Personalized management of long-term conditions: diabetes, hypertension and the rest.

  • Acute care

Immediate care for sudden illness and injury, without waiting for a routine slot.

  • Preventive visits

Health check-ups, kept to schedule rather than left until something goes wrong.

  • Immunizations

  • Hearing and vision examinations

  • Developmental screening for children

  • Orthopedic injury care

What the first three weeks look like

No patient is placed in a program before an assessment, and no assessment happens before we have told you what it will cost.

  1. Days 1–2

    Intake call and benefits check

A coordinator confirms eligibility, verifies your insurance, and schedules your appointment. If you are unable to pay, a sliding-scale discount is applied and you are told the cost before anything is scheduled.

  2. Week 1

    Clinical assessment

A licensed clinician completes the biopsychosocial evaluation, covering history, medications and risk screening, then issues a provisional diagnosis with an ASAM level of care.

  3. Weeks 2–3

    Treatment plan in motion

You start with the prescriber and therapist who will stay with you, and the plan is reviewed at every update as your level of care steps up or down.

What it costs

Settled before you are booked in

You are told what it will cost before anything is scheduled, never after.

  • Sliding scale if you're uninsured

Fees are set against household size and income under the federal poverty guidelines, and the scale is published.
